2.According to the Medical Law in Korea(Medical Service Act Article 21) the patient must come to the hospital in person(or the guardian with the required document to request a copy of the Medical Records.
- The patients: Visit International Healthcare Center with photo ID and complete the “Request Medical
Records Form” - The guardians of the patients(in case the patients can not come to the hospitalin person): The guardians of the patients h ave to come to the International Healthcare Center with their photo ID / copy of photo ID of the patients and proof of authority to act on the patient’s behalf. Please download and fill out the [Consent form for Disclosure and Copy of Medical Records] and [Authorization for Disclosure and Copy of Medical Records] and make sure the patient also fills out this form in the corresponding box and signs the form.

3.The cost of issuance for having the medical records issued free of charge, but if you hold Korean National
Health insurancethe charges will be: 2,000 KRW up to 5 pages, and after 5 pages 200 KRW per pages -
4.Time to issue the medical record will take about 30 – 60 minutes.

5.The Medical Records Office are open from 9am to 6pm, Monday to Friday, and from 9am to 1pm on Saturdays.

6.Neuropsychiatry(Mental Health) Medical Records: The doctor has to issue a “Request Medical Records Form”. Therefore please make an appointment for consultation.
Medical Certificate

1.Please ask the doctor directly for Medical Certificate during the consultation.

2.If you request Medical Certificate after your consultations, we do not request the doctors to write it.
You will have to have one more consultation to request Medical Certificate. -
3.International Healthcare Center do not write English Medical Certificate for the patients.
Please ask your doctor directly during the consultation. -
4.English Medical Certificate: 20,000 KRW / Korean Medical Certificate: 10,000 KRW.
Medical Images Copy: Film Duplications

1.Patients can request a copy of the medical image data at the International Healthcare Center.

2.The medical image data will be copied in the CD, and will take about 30 minutes(may vary depending
on the quantity of data) -
3.Medical Records Office will be open from 9am to 6pm, Monday to Friday, and 9am to 1pm on Saturdays.

4.The cost for copy of Medical Images is free, but if you hold Korean National Health insurance the
charges will be: 19,000 KRW for CD and 25,000 for DVD
Billing Records

1.Visit the International Payment on the 2nd floor of the main building.

2.You can request
- Receipts for your outpatient visits, or inpatient stays.
- Itemized bills for your outpatient visits, or inpatient stays.

3.The patient have to bring their photo ID.

Confirmation of Medical Appointment Letter

1.Visit the International Payment on the 2nd floor of the main building.

2.Confirmation of Medical Appointment letter will ONLY include:
- Hospital ID
- Resident Registration Number
- Patient’s Name
- Doctor’s Name and Specialty
- Date and Time

3.If you need letter with the diagnosis please request Medical Certificate to your doctor during the consultation.
Birth Certificate

1.Please make an appointment at the Gynecology Department under the mother’s name.

2.English Birth Certificate: 20,000 KRW / Korean Medical Certificate: 3,000 KRW.

3.You also have to pay for the consultation fee.
Vaccination Certificate

1.Please make an appointment at the International Healthcare Center (International Patients) or at the Family Medicine Department (Korean National Health insurance Holders).

2.Please bring your notarized vaccination records so our doctors can make reference.
